In order to take advantage of this offer,
it is required that every organization completes a
HEADS UP FOOTBALL AGREEMENT

Completion is simple:  Just download the Agreement from this email and fill out.  This should be done for each organization NOT each team. [For Example: One (1) completed agreement for the LA Lions organization, not three (3) separate agreements for the A, AA, and AAA teams.]  The majority of the terms in the agreement is covered by Louisiana Youth Football and our requirements.  You will only be required to designate a Player Safety Coach for your organization.  This will be addressed at a later date.  Once you have completed the agreement for your organization, simply fax it to the LYSN Office at 225-302-8607 by Thursday, June 13th.
